ALL-STAR PARENT INFORMATION

On Tuesday, June 15 at 6:00 there will be a meeting for all-star parents at each age level.  This meeting should take about 30-45 minutes.

Congratulations on being selected to represent West Nyack this summer in All-Star play.  Being an All-Star is an honor, a responsibility and a commitment.  

Please look under "Forms" on the main page to download the appropriate forms.

Each parent must bring:

- A completed uniform order sheet with cash or a check made out to WNLL.  For players 10 or older with uniforms from last year, a new uniform is not necessary.  Even if you are not ordering a new uniform, please bring an order sheet.

- A player summary form.

(The following items do not apply to 8 & 9 year olds)

- Completed "Medical Release Form."  This form does NOT need to be completed by a physician.

- Completed 2 copies "Proof of Residency Form" with three distinct proofs of residency.  Each proof of residency must be current with the date visible.  Kindly make every effort to make 2 copies with all three proofs visible.  If the name of the parent differs from the name of the child, then further documentation may be needed (i.e. marriage license, adoption papers, etc.)  Any questions regarding proof of residency, email westnyacktwins@yahoo.com

- An original birth certificate for any player who has not played District All-Stars (Districts only include 10, 11, 12 year olds, Juniors and Seniors - Districts do not include 9 or 13 year old teams).

All forms should be given to a designated representative from your team.
